Chamber members are encouraged to send their completed vendor application form, Dept. of Health permit (if required) and Insurance Certificate to Chamber by September 11th deadline.

Please provide us with a Westchester County DOH permit required to serve prepared food.
Vendors serving alcohol must provide us with the special event permit.
All vendors are required to provide an insurance certificate indemnifying the Village of Pleasantville and the Pleasantville Chamber of Commerce for 1M and send to email@example.com.
If your business has a workers comp insurance policy, please email a copy. If it doesn’t, email firstname.lastname@example.org stating that you’re self-employed and don’t have employees.
Vendors must provide a copy of the car insurance policy of the person driving to set up their booth.
Vendors must be present on Washington Avenue no later than 12pm and on Wheeler Avenue, no later than 2pm on the day of the event.
If Vendors are unable to meet any of the insurance requirements, please email the chamber to request a waiver exemption form.
